• Quality of each product
• Payment Method (Visa, MasterCard, American Express, check or money order)

If paying by check or money order your order will be shipped after payment has been received by Baldwin. Incomplete forms will be returned to the employee

Once your order has been received it will be quoted, you will be notified of the total amount due by phone or email.

2. Shipping & Handling:
   1. Credit Card: standard UPS rates apply
   2. Prepaid by check or money order: $3.00 small parts; $5.00 each unit plus an additional $24.00 to ship overnight.
